Our Services


Support coordination is an NDIS funded service that strengthens your ability to coordinate and implement your service providers, mainstream supports and participate in the community.

When Support Coordination is included in your NDIS Plan, you, the participant, can either select your chosen Support Coordinator, or you can allow the NDIS to appoint one. The Support Coordinator then helps you to connect to the other supports on your plan, and manage those relationships as service delivery progresses. The Support Coordinator also reports back to the NDIS and lets them know how your plan is working for you.

Support Coordination assists you to navigate the NDIS, connect with the services you need to fully participate in your community and enhancing your ability to coordinate and manage your own services.

Your Support Coordinator can also help you prepare for your next plan when that time comes around.


Are you currently in hospital waiting to move home after an accident or illness has left you with a permanent disability? Does your home require modification so you can return home, or you now require a new home that is purpose built to support you?

This is where Verbinding can assist you. If you are not already a recipient of an NDIS plan, we can support you in consultation with the hospital staff with your NDIS access request. Our significant experience and networks in disability housing, aged care combined with our NDIS know-how and our connections with experienced OT’s enables us to support you through this process.

Our Hospital to Home (H2H) service will work with you and your family to move you into temporary and safe accommodation until you can safely move back home. This gets you out of hospital while we work hard to speed up getting your home modified or getting you assessed for Specialist Disability Accommodation (SDA) funding from the NDIS so that, if approved for SDA funding, you can move into a new purpose-built home.


Specialist support coordination is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A Specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.  Our very experienced and skilled specialist support coordinators can assist people with multiple and complex support requirements where there are high risk issues and, frequently, very high support needs.


Preparing for your first NDIS plan can be a confusing and overwhelming experience.  It is important to ensure that you receive funding for all of your reasonable and necessary needs related to your disability. We can provide support and consultation to help you prepare for your first planning meeting.
